Does Creatine Cause Hair Loss? What the Research Shows

Creatine has become almost as common in the gym as dumbbells and bad playlists. But for guys who care about keeping their hair, one question keeps following it around:

Does creatine cause hair loss?

Based on the human research we have today, there is no good evidence that creatine causes or accelerates hair loss. In fact, the first randomized controlled trial to directly measure hair after creatine supplementation found no negative effect on hair density, thickness, follicle count, or other measures of hair growth.[1]

But that doesn't mean the question is ridiculous—or that science can give you a lifetime guarantee.

The concern can be traced largely to one small study from 2009 that found an increase in dihydrotestosterone (DHT) after men took creatine.[2] DHT plays an important role in male pattern hair loss, so the finding understandably raised some eyebrows.

The problem?

That study never measured hair loss.

Sixteen years later, researchers finally did.

Here's what we actually know.

Where Did the Creatine and Hair Loss Concern Come From?

The creatine-hair-loss story begins with a 2009 study involving 20 college-aged male rugby players.[2]

Researchers used a relatively aggressive creatine regimen: 25 grams per day for seven days as a loading phase, followed by 5 grams per day for another 14 days.

Testosterone didn't significantly change.

DHT did.

After the seven-day loading phase, DHT was 56% higher than baseline. After another two weeks at the lower maintenance dose, it remained about 40% above baseline.[2]

Those percentages sound dramatic. And if you've spent any time researching male pattern baldness, you can probably see how the next step happened:

Creatine increased DHT.

DHT is involved in male pattern baldness.

Therefore, creatine causes hair loss.

Except the researchers never demonstrated that last part.

They measured testosterone and circulating DHT. They did not measure hair density, shedding, follicular miniaturization, or baldness.

A later review also pointed out some useful context: DHT was lower at baseline during the creatine condition than during placebo, the absolute DHT concentrations remained within normal clinical ranges, and the result has not been consistently reproduced in subsequent creatine research.[3]

In other words, the 2009 study raised a reasonable question.

The internet gradually turned that question into an answer.

What Does DHT Actually Have to Do With Male Pattern Hair Loss?

To understand why the 2009 study attracted so much attention, it helps to understand what DHT actually does.

Your body converts some testosterone into DHT using an enzyme called 5-alpha-reductase.

In men who are genetically susceptible to androgenetic alopecia, better known as male pattern hair loss, androgen activity contributes to a process called follicular miniaturization. Over repeated hair-growth cycles, susceptible follicles produce progressively finer and shorter hairs.[4][5]

Eventually, some follicles may stop producing cosmetically visible hair.

This is also why medications such as finasteride reduce DHT as part of their approach to treating male pattern hair loss.

But there's an important distinction that often gets lost online:

More DHT in Your Bloodstream Isn't the Same Thing as Proving That You'll Lose More Hair

Male pattern hair loss depends heavily on genetic susceptibility and how individual scalp follicles respond to androgens. In fact, normal circulating androgen levels can be enough to drive hair loss in genetically susceptible men.[4]

Research into androgenetic alopecia also points to differences in androgen-receptor activity and other biological processes within susceptible scalp follicles.[5]

So the biology is considerably more complicated than:

More blood DHT = more baldness.

That's why a study showing a change in serum DHT can generate an interesting hypothesis—but it doesn't demonstrate an actual hair-loss outcome.

Did Other Creatine Studies Find the Same DHT Increase?

Not consistently.

A 2021 review examining common questions and misconceptions surrounding creatine found that the 2009 rugby study remained the primary source behind the creatine-hair-loss concern.[3]

The review identified 12 other studies examining creatine and testosterone. Most found no change in total testosterone, and studies that measured free testosterone generally did not find increases either.[3]

That doesn't prove creatine could never affect DHT under any circumstances.

But it does mean the original hormonal finding never developed into a large, consistent body of evidence showing that creatine raises androgens.

There was still an obvious problem, however:

Most of this research wasn't actually studying hair.

For years, the most accurate answer to “Does creatine make you lose your hair?” was essentially:

We don't have evidence that it does—but nobody has directly tested the question very well either.

“In summary, the current body of evidence does not indicate that creatine supplementation increases total testosterone, free testosterone, DHT or causes hair loss/baldness.”

That changed in 2025.

Researchers Finally Studied Creatine and Actual Hair

In 2025, researchers published the first randomized controlled trial designed specifically to examine creatine supplementation and hair health.[1]

They recruited 45 resistance-trained men between ages 18 and 40 and randomly assigned them to take either:

  • 5 grams of creatine monohydrate per day, or
  • 5 grams of a maltodextrin placebo per day.

The trial lasted 12 weeks, and 38 participants completed it.[1]

Researchers measured total testosterone, free testosterone, and DHT.

But, importantly, they also measured the thing previous creatine research hadn't:

Their hair.

Using standardized hair assessments, researchers examined measures including:

  • total hair count
  • hair density
  • percentage of hairs in the growth and resting phases
  • follicular unit count
  • terminal and vellus hairs
  • cumulative hair thickness

The researchers found no significant differences between the creatine and placebo groups in DHT or in any of the measured hair outcomes.[1]

Put more simply:

After 12 weeks, the men taking creatine did not have measurably worse hair than the men taking the placebo.

That's considerably stronger evidence than looking at a hormone measurement and speculating about what might eventually happen to someone's scalp.

Does the 2025 Study Prove Creatine Can't Cause Hair Loss?

No—and this is where the answer needs more restraint than you'll find in some headlines.

The 2025 study is reassuring.

It's also one relatively small, short-term trial.

It Lasted Only 12 Weeks

Male pattern hair loss develops gradually, often over years.

A 12-week trial can detect changes during that period, but it cannot tell us with certainty whether taking creatine for five, ten, or twenty years might affect hair in some men.

The researchers themselves acknowledged that longer-duration and higher-dose studies could provide additional information.[1]

The Study Was Modest in Size

Forty-five men were randomized and 38 completed the trial.[1]

That's enough to provide useful evidence, but it isn't the kind of large, long-term dataset that would allow researchers to rule out every small or uncommon effect.

The Participants Were Young, Resistance-Trained Men

That's actually a sensible population to study because plenty of creatine users fit that description.

It doesn't necessarily tell us what would happen in every population, though—particularly older men who are already experiencing significant androgenetic alopecia.

Researchers Didn't Record Family History of Hair Loss

This may be the most interesting unanswered question for men who already know baldness runs in the family.

Androgenetic alopecia has a strong genetic component.[4][5]

The 2025 researchers specifically acknowledged that they did not assess family history of hair loss.[1]

That means the study wasn't designed to determine whether men with a particularly strong genetic predisposition might respond differently over a much longer period.

They Measured DHT in the Blood—Not Androgen Activity Within Scalp Follicles

The trial measured circulating hormones using blood samples.

But androgenetic alopecia involves biological activity occurring locally within genetically susceptible hair follicles. The authors noted that future research could examine androgen levels more directly in hair.[1]

They Didn't Use a Creatine Loading Phase

Participants took 5 grams per day throughout the 2025 study.[1]

That's different from the 2009 study, which started participants at 25 grams per day for seven days before reducing the dose to 5 grams.[2]

That's worth noting because the largest DHT increase in the original study occurred after the loading phase.

None of these limitations mean the 2025 study should be dismissed.

They simply define what we can—and can't—reasonably conclude from it.

One Additional Disclosure Worth Knowing

The study reported no direct research funding. However, the authors acknowledged support from the official representative of BPI Sports USA, and several authors disclosed advisory, consulting, research, or other professional relationships with sports-nutrition and supplement companies or organizations.[1]

Those relationships don't invalidate the results. They are simply relevant context when evaluating the evidence, particularly when this is still the only randomized trial directly examining creatine and hair outcomes.

What If Male Pattern Baldness Runs in Your Family?

This is probably the most useful unanswered question for the guy staring at a tub of creatine and his father's hairline.

A family history of androgenetic alopecia can indicate greater genetic susceptibility—but inheritance is complicated and involves many genes. There isn't one simple “baldness gene” passed down from one particular side of the family.[4][5]

More importantly:

Having a genetic predisposition doesn't mean creatine has been shown to accelerate it.

Current human evidence has not demonstrated that creatine speeds up androgenetic alopecia in genetically susceptible men.

At the same time, the 2025 trial didn't specifically study genetically high-risk men over multiple years.

That's an important distinction:

“We haven't found evidence that creatine causes hair loss” is not the same statement as “we've proved that it could never affect anyone under any circumstances.”

The evidence today is reassuring.

It simply isn't exhaustive.

What If You Started Losing Hair After Starting Creatine?

This is where things get tricky.

Say you start taking creatine in January. A few months later, you notice more scalp showing through your hair.

It's natural to connect the two.

But timing alone doesn't prove that one caused the other.

Male pattern hair loss is extremely common and progressive. Increased shedding can also occur after major illness, significant psychological or physical stress, rapid weight loss, crash dieting, nutritional problems, certain medications, and other physiological changes.[4][6]

We've written about a similar issue with testosterone replacement therapy: hormones and hair biology are related, but the relationship isn't as simple as blaming every change in your hairline on the newest thing you've introduced into your routine.

If you're noticing legitimate thinning, the more useful question may not be:

“Should I throw away my creatine?”

It's:

“What kind of hair loss am I actually experiencing?”

A gradually receding hairline or thinning through the crown may fit the pattern of androgenetic alopecia. Sudden, more diffuse shedding may point toward something different, such as telogen effluvium.[4][6]

Getting the cause right matters more than automatically blaming the scoop in your gym bag.

Should You Stop Taking Creatine If You're Worried About Your Hair?

Current evidence does not establish that men concerned about hair loss need to stop taking creatine solely because of their hair.

The strongest direct human study we currently have found no measurable adverse effect on hair after 12 weeks of taking 5 grams of creatine per day.[1]

That doesn't mean you should ignore a genuine change in your hair.

If you're noticing new thinning or increased shedding, consider documenting it consistently—similar lighting, hairstyle, and camera angles—and discuss it with an appropriate healthcare professional.

The goal is to determine what's actually happening rather than assuming creatine is responsible.

That's considerably more informative than counting a few hairs in the shower after leg day.

The Bottom Line on Creatine and Hair Loss

Creatine's reputation for causing hair loss got ahead of the evidence.

The concern largely originated with a small 2009 study that found an increase in circulating DHT after creatine supplementation.[2] Because DHT plays an established role in male pattern hair loss, the finding created a plausible question.

But that study never measured hair loss.

The first randomized controlled trial to actually examine hair outcomes was published in 2025. Men taking 5 grams of creatine per day for 12 weeks showed no significant differences in DHT or measured hair outcomes compared with men taking a placebo.[1]

That's reassuring—and currently the best direct evidence we have.

But it isn't a lifetime guarantee.

The study was modest in size, relatively short, and didn't specifically examine men according to their genetic predisposition to male pattern hair loss.

So where does that leave us?

Current evidence does not show that creatine causes hair loss. The theory is considerably stronger on the internet than it is in the scientific literature. But longer-term research—especially in men already predisposed to androgenetic alopecia—would make that conclusion more certain.

For now, if you're watching your hairline and taking creatine, there's probably more value in understanding what is actually causing your hair loss than automatically blaming the scoop in your gym bag.

Frequently Asked Questions

Does Creatine Increase DHT?

One small 2009 study involving male rugby players found that DHT increased after creatine supplementation, particularly following a seven-day loading phase.[2]

However, the result has not been consistently replicated. A 2021 review concluded that the broader creatine literature did not support a consistent increase in testosterone or DHT, and a 2025 randomized trial found no significant difference in DHT between men taking 5 grams of creatine per day and those taking a placebo.[1][3]

Does Creatine Cause Hair Loss If Baldness Runs in My Family?

There is currently no direct evidence showing that creatine accelerates male pattern hair loss in men with a family history of baldness.

However, the 2025 trial did not assess participants according to family history, and longer-term studies specifically involving genetically susceptible men would help answer this question more confidently.[1]

Is Hair Loss From Creatine Reversible?

We don't currently have good evidence that creatine causes hair loss in the first place, so there isn't an established medical condition that could accurately be called “creatine-induced hair loss.”

If hair thinning begins while you're taking creatine, identifying the actual type and cause of hair loss is more useful than assuming the supplement is responsible.

Should I Stop Taking Creatine If I'm Already Losing My Hair?

Current research does not establish that men experiencing hair loss need to stop taking creatine solely because of concerns about baldness.

If you're noticing a receding hairline, crown thinning, or unusual shedding, determining whether you're experiencing androgenetic alopecia, telogen effluvium, or another type of hair loss will usually provide more useful information.
