Men’s Hair Loss: Causes, Early Signs, Treatments & What Actually Works
Maybe you noticed it in a photo first. Your hairline looks a little higher. The corners have crept backward. The crown catches more light. Or your hair simply doesn’t style the way it did a couple of years ago.
Then comes the obvious question: Am I actually losing my hair?
There isn’t one universal answer. Men lose hair for different reasons, and those reasons matter. Progressive male pattern hair loss, temporary shedding after rapid weight loss, and an inflammatory scalp condition are very different problems.
The useful way to think about hair restoration is not to start with a product. Start with what is happening. Are you trying to preserve viable hair, improve thinning hair, or replace coverage that has already been lost? Once you know that, the long list of hair-loss treatments becomes much easier to understand.
Hair Shedding vs. Progressive Hair Loss: They’re Not the Same Thing

Seeing hair in the shower does not automatically mean you are going bald.
Hair naturally moves through repeating phases of growth, rest, and shedding. The American Academy of Dermatology (AAD) notes that shedding roughly 50 to 100 hairs a day can be normal. Sometimes, however, a larger number of hairs enter the resting phase and then shed over a relatively short period. This is called telogen effluvium. [2]
Common triggers include illness, high fever, surgery, major physical or emotional stress, significant weight loss, and inadequate calorie or nutrient intake. Noticeable shedding may not begin until several months after the trigger. [2]
Male pattern hair loss usually looks different. Men are more likely to notice a receding hairline, thinning crown, or gradual loss of density over time. [3]
The two can also overlap. A man predisposed to male pattern hair loss can still experience temporary excessive shedding. That is why counting hairs in the sink rarely tells the whole story. Pattern, timing, scalp findings, and change over time are more useful clues.

Male Pattern Hair Loss: What’s Actually Happening
The most common cause of hair loss in men is androgenetic alopecia, better known as male pattern hair loss or male pattern baldness.
Genetics play a major role, but the biology is more complicated than simply having “too much DHT.” Dihydrotestosterone, or DHT, is an androgen produced from testosterone. In genetically susceptible scalp follicles, androgen signaling—particularly involving DHT—contributes to progressive follicular miniaturization. Over repeated growth cycles, affected follicles produce hairs that become shorter, finer, and less visible. [3] [4]
So hair loss often progresses gradually rather than switching suddenly from “hair” to “no hair.” That distinction matters: a follicle still producing a thin, miniaturized hair presents a different treatment opportunity than an area that has been bald for many years.
Common early signs include recession at the temples, a changing frontal hairline, thinning at the crown, reduced density across the top of the scalp, and increasing numbers of finer, shorter hairs. [3]
The Norwood-Hamilton scale is commonly used to describe the progression of male pattern hair loss. You do not need to memorize the stages or diagnose yourself from a chart. Its practical value is showing that early thinning and advanced baldness represent very different stages.
Comparable photographs taken several months apart can also be useful. Gradual change is hard to recognize when you see yourself every day. And when progressive male pattern hair loss is present, earlier treatment generally gives you more hair to preserve.

Other Causes of Hair Loss in Men
Male pattern hair loss is common, but genetics should not become the default explanation for every change in a man’s hair.
Hair loss or excessive shedding can also be associated with thyroid disease, nutritional deficiencies, autoimmune conditions such as alopecia areata, certain medications, severe illness, rapid weight loss or significant calorie restriction, and inflammatory or scarring scalp disorders. [6]
Hormonal and metabolic changes can complicate the picture as well. Because male pattern hair loss is androgen-dependent, men using testosterone replacement therapy often have questions about whether changing androgen exposure could affect genetically susceptible follicles. Likewise, recent systematic reviews have found an association between GLP-1 receptor agonists and reported hair loss, but the mechanism is still being studied. Rapid weight loss, reduced nutritional intake, and the physiologic stress of substantial weight change are among the possible contributors. [7] For a deeper look, see GLP-1s and Hair Loss.

Sudden loss, distinct bald patches, significant scalp pain or inflammation, scaling, scarring, or unusually rapid progression should prompt medical evaluation rather than trial-and-error treatment. [6]
What Can Hair Loss Treatment Realistically Do?
Hair restoration becomes easier to understand when you separate three different goals.
Preserve viable hair. Slow or stabilize additional loss.
Improve viable hair. Help miniaturized follicles produce stronger, more visible hair.
Replace lost coverage. Restore hair to areas where useful coverage is no longer present.
Those goals can overlap, but they are not interchangeable.
For some men, preventing further progression is an excellent result. Others may see meaningful improvement in density. Men with advanced, longstanding baldness may ultimately need transplantation if the goal is to recreate substantial coverage. [5] [22]
This is also why the phrase “hair growth” can be misleading. A measurable increase in hair count is not necessarily the same thing as a dramatic cosmetic transformation.

Hair Loss Treatments for Men: What Actually Works?
There is no shortage of products promising to grow hair. The evidence behind them varies considerably.
Minoxidil
Topical minoxidil is one of the most established treatments for male pattern hair loss and is FDA-approved as an over-the-counter hair-regrowth treatment for men. [8]
Minoxidil does not block DHT. Instead, it influences follicular activity and the hair-growth cycle. Some men experience reduced loss and improved density or thickness, while others respond more modestly. Benefits generally require continued use. [5]
FDA labeling for common men’s 5% topical minoxidil products is for thinning on the vertex, or top of the scalp. The label states that these products are not intended for frontal baldness or a receding hairline. Minoxidil may be used more broadly in clinical practice, but that is an important distinction between the labeled indication and wider use. [8]
Oral minoxidil is also increasingly prescribed for hair loss, but this use is off-label in the United States. A randomized clinical trial in men found that 5 mg oral minoxidil was not superior overall to 5% topical minoxidil after 24 weeks, although some photographic outcomes favored oral treatment at the vertex. [9]
Because oral minoxidil is a systemic vasodilator and can affect cardiovascular function and fluid balance, it should be used with appropriate medical oversight. [10]
Finasteride and Dutasteride
Finasteride addresses one of the major biological drivers of male pattern hair loss. It inhibits type II 5-alpha-reductase, an enzyme involved in converting testosterone to DHT. At 1 mg daily, finasteride is FDA-approved for male pattern hair loss in men. [11]
One detail is especially relevant for men concerned about their temples: the prescribing information states that efficacy in bitemporal recession has not been established. [11]
Sexual side effects—including decreased libido, erectile dysfunction, and ejaculation disorders—are recognized adverse reactions and deserve a balanced discussion rather than either dismissal or alarmism. [11]

Dutasteride inhibits both type I and type II 5-alpha-reductase and suppresses DHT more broadly. Research suggests it can be highly effective for androgenetic alopecia, with some comparative studies showing stronger hair-growth outcomes than finasteride. [13]
However, dutasteride is not FDA-approved in the United States for hair loss. Its approved indication is benign prostatic hyperplasia, so its use for androgenetic alopecia is off-label. [12]
Low-Level Light and Red-Light Therapy
Low-level light therapy—often called LLLT or photobiomodulation—is a non-drug treatment supported by a meaningful body of research.
A 2024 systematic review and meta-analysis that included 38 studies and more than 3,000 patients found significant improvements in hair density with LLLT versus placebo among patients with androgenetic alopecia. [14] Randomized sham-controlled trials have also reported improvements in measures such as hair count, thickness, and coverage. [15]
That makes LLLT a legitimate nonsurgical option, particularly for men who want a non-pharmaceutical modality or who are combining several approaches.
There is an important regulatory distinction. The FDA has a Class II device category for certain hair-growth laser devices intended to promote hair growth in men with androgenetic alopecia, and individual devices can receive 510(k) clearance. [16] A 510(k) clearance is not the same thing as FDA approval of a medication; the process evaluates whether a device is substantially equivalent to a legally marketed predicate device. [17]
The evidence comes from specific devices and treatment protocols—not from red light as a generic category. Devices studied for hair growth use defined wavelengths, energy delivery, treatment schedules, and scalp coverage. A generic red-light panel or red-light sauna should not automatically be considered equivalent to a purpose-built hair-growth device.

Platelet-Rich Plasma (PRP)
PRP uses a patient’s own blood. The blood is processed to concentrate platelets, and the resulting plasma is injected into the scalp.
Randomized trials and meta-analyses have found improvements in hair density among patients with androgenetic alopecia. The major limitation is standardization: platelet concentration, preparation methods, injection techniques, and treatment schedules vary considerably among studies. [18]
A 2024 meta-analysis found increased hair density but rated the underlying evidence as low quality because of substantial heterogeneity and publication bias. [18]
So PRP is best described as an evidence-supported treatment with encouraging results, but with more variability than established therapies such as finasteride and topical minoxidil.
Flycatcher does not offer PRP. Our hair-restoration programs use non-invasive modalities instead, but PRP remains a legitimate option for men comparing treatments.
Microneedling
Microneedling creates controlled micro-injuries in the scalp. In hair restoration, it is thought to influence wound-healing pathways and may enhance the effects of other treatments.
The strongest evidence currently supports it as an adjunct, particularly alongside minoxidil. A 2025 systematic review and meta-analysis of randomized trials found that microneedling combined with minoxidil produced greater improvements in hair count and diameter than minoxidil alone, although the hair-count studies showed substantial heterogeneity. [19]
That does not mean more aggressive needling is better. Protocols are not fully standardized, and excessive home treatment can cause irritation and other problems.
Supplements, Shampoos and “Natural” Treatments
This is where hair-loss marketing gets especially noisy.
The most useful distinction is simple: correcting an actual nutritional deficiency is different from taking a hair supplement when you are not deficient.
Deficiencies in nutrients such as iron, zinc, biotin, or protein can contribute to hair loss. The AAD advises that supplements such as biotin, iron, or zinc should be used when testing shows a deficiency; unnecessary or excessive supplementation can be harmful. [20]
Some specific supplement formulations have been studied, but evidence for one formulation should not automatically be applied to every “hair growth” supplement.
Shampoo deserves similar perspective. Treating dandruff, seborrheic dermatitis, and other scalp conditions can be useful. Ketoconazole has also shown enough potential in androgenetic alopecia to warrant study as an adjunct, although stronger randomized trials are still needed. [21]
A shampoo can support scalp health. It should not be mistaken for a treatment capable of reversing genetically driven follicular miniaturization on its own.
Hair Transplants
Hair transplantation solves a different problem from the nonsurgical treatments above.
A surgeon takes healthy follicles from a donor area—usually the back or sides of the scalp—and places them into areas where coverage has been lost. Modern techniques can produce natural-looking, durable results in appropriate candidates. [22]
But transplantation does not stop androgenetic alopecia. Native, genetically susceptible hair can continue thinning even after transplanted follicles are placed. A good candidate also needs adequate donor hair, and men with rapidly progressing loss may sometimes benefit from stabilizing that progression before surgery. [22]
Flycatcher does not perform hair transplants. For men with advanced loss, however, surgery may offer a degree of coverage that nonsurgical treatment cannot realistically reproduce.

Why Combination Therapy Often Makes More Sense
The question is often framed as: Which treatment is best?
A better question may be: Which treatments address different parts of my problem?
Finasteride or dutasteride target androgen-related progression. Minoxidil stimulates follicular growth. Low-level light therapy offers a non-drug stimulatory approach. Microneedling may strengthen results as an adjunct. A transplant solves a different problem entirely: missing coverage.
Research supports several combinations. A meta-analysis of randomized trials found that finasteride plus topical minoxidil performed better than monotherapy on several global outcome measures, although not every measured endpoint favored the combination. [23] Microneedling plus minoxidil also has supportive randomized-trial evidence. [19]
But more treatment is not automatically better treatment. A good combination has a reason behind each component and still has to make sense in terms of side effects, cost, convenience, and your likelihood of sticking with it.
How Long Do Hair Loss Treatments Take to Work?
Hair grows slowly. Meaningful change usually takes months, not weeks.
- 0–3 months: generally too early to judge cosmetic success.
- 3–6 months: stabilization or early changes may become noticeable.
- 6–12 months: often a more useful window for evaluating the overall response to nonsurgical treatment.
Those are rough guideposts, not promises. Different treatments have different timelines, and individual response varies. The AAD notes that minoxidil may take six to twelve months for some men to show results, while finasteride prescribing information says daily use for three months or more is generally necessary before benefit is observed. [5] [11]
Progress is best judged with consistent photographs or objective hair and scalp measurements rather than memory alone. For treatments such as minoxidil and finasteride, continued use is generally required to maintain treatment-dependent benefits. [5]

How to Choose the Right Hair Loss Strategy
A 28-year-old whose temples have just started receding and a 52-year-old with a large bald crown can both search for “the best hair loss treatment.” They probably should not receive the same plan.
A useful strategy starts with a few basic questions:
- What type of hair loss is occurring?
- How much viable hair remains?
- Is the priority slowing loss, improving density, or restoring missing coverage?
- Are you comfortable using prescription medication?
- How invasive do you want treatment to be?
- What can you realistically maintain long term?
Consider three examples.
A man with early progressive recession may primarily need a preservation strategy.
A man who begins shedding heavily several months after losing 40 pounds may need to investigate telogen effluvium, nutrition, and the timing of the weight loss before assuming he has suddenly developed aggressive male pattern baldness.
A man with a longstanding bald area may still benefit from protecting his remaining hair, but transplantation may be the more realistic way to restore substantial coverage.
There is no virtue in choosing the most aggressive treatment. The goal is a strategy that matches the biology, your goals, and what you can realistically maintain.

Questions to Ask Before Starting Hair Loss Treatment
A good consultation should leave you understanding why a treatment is being recommended.
- What type of hair loss do you believe I have?
- What signs led you to that conclusion?
- Are we trying to preserve hair, improve density, replace coverage—or some combination?
- What evidence supports the treatment you are recommending?
- Is it FDA-approved, FDA-cleared, or being used off-label?
- What is realistic at my current stage of hair loss?
- How will progress be measured?
- When should we decide whether it is working?
- What side effects or maintenance requirements should I understand?
- What happens if I stop?
- When would you tell me non-surgical treatment probably will not accomplish enough?
That last question matters. A provider offering nonsurgical hair restoration should still be willing to tell you when surgery or another approach may better match your goals.
The Bottom Line on Men’s Hair Loss
Hair loss gets easier to understand when you stop searching for one miracle product.
First, determine what is happening. Then decide whether the job is to preserve viable hair, improve what is still growing, or replace coverage that has already been lost.
For many men with androgenetic alopecia, treatments such as minoxidil, finasteride, and properly designed low-level light therapy can play meaningful roles. Combination strategies may make sense because different treatments address different mechanisms. PRP and microneedling have encouraging evidence but greater variability. Supplements are most useful when they correct an actual problem. And when substantial coverage is already gone, transplantation deserves an honest place in the conversation.
There is no treatment that works equally well for every man, every cause, or every stage. But there are more legitimate options than there used to be—and understanding the problem early gives you more of them.
